I know how the Hulk can die, from my opinion hulk can die easily when he was in human form. Trust me, you will think that what Bruce Banner said in Avenger is true that he can't die :.
From what I can think of, Bruce Banner still can die, but in a crazy way. What if Bruce Banner in human form exploded his head with a grenade or even with c4?

Plus you are mixing canons by referencing the comics and the MCU which do differ. I'd suggest checking out the Tour to understand how to better ask and answer questions here. That's kind of Cap's modus operandi- he never gives up, no matter how powerful his opponent or how badly Steve Rogers gets hurt.
That does mean, though, that he's going to take some losses and that's exactly what would happen if he went up against the Hulk on his own. Cap's good but him losing to the Hulk is inevitable and even he would know it.
The Vision started out as a servant of Ultron but bucked his programming and joined up with the team. He's been an Avenger for a very long time and has faced down the Hulk before, so he knows what to expect. While there are some who would argue that Vision couldn't the Hulk on his own, the Vision definitely has the power to take Hulk down. He can hit the Hulk form long range with his solar beams, his flight gives him mobility and speed that would be hard for the Hulk to match, but what will really give him the leg up is his density control.
He can go intangible, keeping the Hulk from striking him and then up his density, making his super strong shots hit even harder. It might take some time and effort but Vision would be able to take the Hulk down. Wonder Man is one of those Avengers who is way tougher than people give him credit for but he doesn't get the love that other members get. Given amazing strength and durability as well as the power to fly from ionic energy experiments, he's hard to keep down- he's died multiple times but always comes back because of his status as a being made of ionic energy.
Wonder Man would put up a great fight against Hulk but eventually, things would get out of hand for him. He would be able to punch it out with the Hulk for a while but he's not strong enough to take him out before things get out of hand and the Hulk's strength starts to really ramp up.
Hulk would quickly get to be too much for him to handle. Scarlet Witch is another Avenger who has faced the Hulk a lot over the years. However, back in those days, she wasn't nearly as powerful as she is now. Her power to manipulate reality makes her a match for even the most powerful cosmic beings and while she is just a normal woman durability wise, she knows enough about fighting the Hulk that she's not going to mess around.
Scarlet Witch doesn't like to go all out with her powers too much but she'll definitely make an exception against the Hulk. He's just too dangerous for her to mess around with, so she'll do what she has to do take him out as quickly as possible. Black Panther is one of the most formidable heroes in the Marvel Universe. He's pretty much the total package- skilled, intelligent, and the leader of his own country.
